Lumeris, a value-based care enablement company, announced that its participation in the CMS Accountable Care Organization Realizing Equity, Access, and Community Health (ACO REACH) model resulted in $22.1 million in savings for the Medicare Trust Fund in 2023. Lumeris achieved gross savings of $89.41 per member per month, an 8.1 percent savings rate compared to CMS benchmarks 26.5 percent higher than other ACOs. Lumeris’ affiliated ACOs maintained a high standard of care, achieving a top-quality score of 99.63 percent, underscoring their ability to balance cost efficiency with superior care delivery.
These outcomes were driven by Lumeris’ AI-powered technology platform, which provided real-time data insights, predictive analytics, and point-of-care workflows to improve population health management. The platform enabled proactive interventions, reducing hospital readmissions and avoidable admissions. Since joining the ACO REACH program in 2022, Lumeris has generated $98.8 million in gross savings, including $56.6 million for the Medicare Trust Fund. The organization also partners with other Medicare programs, including Medicare Advantage and the Medicare Shared Savings Program, to expand its impact. The ACO REACH model emphasizes equity, access, and cost control while incentivizing organizations to improve patient outcomes, reinforcing Lumeris’ commitment to advancing value-based care strategies.
