Forcura, an intelligent workflow management company, and Medalogix, a clinical decision support company, have merged to create an advanced post-acute care technology platform. This collaboration aims to improve patient care for home-based providers and across the broader healthcare system. The combined organization will be majority-owned by Berkshire Partners, with The Vistria Group as the largest minority shareholder. By integrating their technologies, the new platform will help providers manage patient transitions, optimize resource use, improve care coordination, and enhance end-of-life care. The platform will also use artificial intelligence to refine patient care delivery and provide greater transparency for payers and referral sources.
The merger seeks to address challenges in the post-acute care sector, which often operates in isolation from other healthcare services. The new platform will better connect post-acute care providers with the larger healthcare ecosystem and support fair reimbursement models as the industry moves toward value-based care.
