Chordline Health, a cloud-based technology to improve efficiency and patient care using relevant, real-time data, has launched its ACUITYnxt Clinical Care Management Programs, leveraging advanced analytics and automation to improve the identification and management of chronic diseases. The programs focus on using patient and population-level data to identify individuals overdue for health risk assessments or those at risk for conditions including diabetes, asthma, COPD, heart failure, coronary artery disease, and pregnancy-related complications. By enabling earlier intervention, the programs aim to reduce the progression of chronic illnesses, enhance patient outcomes, and lower healthcare costs. Patient engagement tools are a core feature, empowering individuals to adhere to recommended treatments and take an active role in managing their health.
ACUITYnxt integrates insights from an organization’s technology ecosystem, providing a comprehensive view of patient health and social determinants of health. It uses risk stratification to help clinicians proactively address barriers to care and identify patients needing care coordination. Billie Jo Nutter, CEO of Chordline Health, emphasized the importance of preventative care in improving long-term health outcomes and reducing expenses. “We know that earlier interventions keep people healthier for longer and can dramatically lower healthcare costs for all parties. Chordline’s new Clinical Care Management Programs put critical information about risk factors right into the hands of payers and providers so they can make the right clinical decisions now before a chronic condition worsens,” he said.
